Lots of excuses can be made for bad eating when short on time, on the go, travelling etc but these most of the time ARE excuses, not reasons.
With the proliferation of 'local' supermarkets offering a wide range of stuff, much of which is pre-prepared, you have many healthy choices.
Here is an example meal I have just eaten that requires NO preparation.

I read that so much in your posts, I go to bed dreaming about it
You're right though - the more I look at the food in supermarkets with a nutritionally focussed mind, the more I realise how much is procesed rubbish, which leaves the clean food (as pictured) very much the obvious choice.

Well, I'm lazy, and if I had to cook every meal it'd be hours a day taken up!
Thing is even with those excuses there is still a healthy option.
And yep, things that are as close as possible to how they came out of the ground or walked about are always the healthiest.

I'm sure I read a John Beradi(?) article where he said he would often have the same breakfast and lunch for a week, varying only the evening meal, before swapping to a different breakfast and lunch all week the following week.
Like you say - it is possible to be lazy and healthy with minimal planning.

Indeed I think there's a lot to be said for eating the same things routinely - people complain about it but when you ask them to list all the meals they eat regularly there's normally under 10 anyway, they're just normally all crap!
